Application Procedure for Endorsement/Correction on the Passport

 

      A.  Endorsement of child on mother’s passport 

1.  Fill up the application from with your signature where necessary.

2.  Apply with birth certificate for endorsing children’s name in the mother’s passport.

3.  Two names of the children under age 12 can be endorsed in the mother’s passport.

4.  Submit the filled in forms with one copy of passport size and stamp size photographs

5.  Submit the CPR copies of the parent

6.  Please pay the fees in cash

7.  Normal Fees is BD 4.400 per endorsement

8.  Urgent Fees is BD 8.400 per endorsement 

B.  Correction/Change of Name/Father’s Name/Husband’s Name 

1. Submit an affidavit duly attested by Foreign Ministry at Dhaka regarding necessary correction or change in the passport.

2. Submit the duly attested marriage certificate/Kabin Nama/Talak Nama by the Consular Section of the Foreign Ministry at

    Dhaka/at Bahrain (whichever is applicable) for endorsing/changing the husband’s name on the passport.

C. Correction/Change of Profession 

     Submit with the letter from the sponsor along with the CPR copy of the sponsor

D.  Change of Date of Birth: 

     Submit the SSC certificate, duly attested by the Controller of the Education Board and the Consular Section of the Foreign

     Ministry. In case of non-matriculate national, submit the notarized birth certificate or affidavit duly attested by the

     Consular Section of the Foreign Ministry at Dhaka.

E.  Correction/Change of Permanent Address: 

     Submit an affidavit or certificate from the Chairman or Ward Commissioner duly attested by the Consular Section of the

     Foreign Ministry at Dhaka.
